Birth Certificate Copies of birth certificates are only available to the person named on the certificate (18 years or older), the person's parents, a lawful representative, or anyone who presents a court order. If you were born within the city limits of White Plains (St. Agnes Hospital, White Plains Hospital, or at home) and are over 18 years of age, you may obtain a certified copy of your birth certificate. If another situation applies, please contact our office at 914-422-1227 for additional requirements.
How to Apply: In person: Bring the appropriate identification for the type of certificate you are requesting, along with $10 fee (either cash or money order), and a completed application (which you can download from this page) or which will be provided when you come in to the office.
By mail: Enclose a photocopy of the appropriate identification for the type of certificate you are requesting, the required fee of $10.00 per certificate in the form of a money order/certified check made payable to "The City of White Plains". (We do not accept personal checks), and a completed application (which you can download from this page) and mail to: Office of City Clerk, 255 Main Street, White Plains, New York 10601. Requests are usually answered the same day in which they are received.
Identification: One of the following is required; A Current Driver's License, Non-Drivers Identification, Passport, Military ID, Two (2) different utility bills of applicant, or a police report of lost/stolen ID.
